Meet Tonya Hubbs, RN

FOUNDER OF 904 SENIOR CARE SOLUTIONS

Tonya Hubbs is a registered nurse with decades of experience in both hospital and in-home settings. She has had the privilege of serving in various roles working across disciplines—from acute care to home health—and brings a comprehensive understanding of how to navigate the healthcare system with both efficiency and compassion. She became a champion for navigating, educating, and assisting her patients to take on their health challenges to improve their lives independently.

With expertise in multiple areas, it gave Tonya a bird’s eye view of the healthcare system which challenged her to find ways to assist patients/families on how to advocate for what they needed and deserved. Tonya identified one thing that was persistent with her patients, aging at home. With a little bit of ingenuity and persistence she was able to assist her patients/families to achieve their goals. Helping her own parents ages 91 and 93 still living at home with a little assistance, ignited this passion and desire to build a business that supports seniors and their families on this journey.

Her approach combines clinical training with deep compassion, ensuring every care plan is built not just around medical needs, but real life.

Her work is guided by Proverbs 3:27:

That servant-hearted philosophy shapes every interaction—with seniors, families, and the care professionals she collaborates with.
